This component is very close to what I need but it is missing just a few minor things.  Here they are:

Fields that only a admin can change - this could probably be done with a simple admin switch to hide fields to the users so they never see the data.  You could probably make the switch slightly more sophisticated by allowing three states: editable, viewable (aka locked), and hidden.

Field Categorizing
It would be nice to be able to do tabbing like Community Builder does.  This probably would mean too much rewriting of the core Joomla stuff to do easily but it would be nice (cause it's pretty cool in CB).

What might be simple is to do is allow "separator" fields which would act as a title bar and separator.  If you took a class as well as the title text you could put the text in a <div> (inside the <td>) and let us fashion it (including sizing, color, margins,etc.).

For me the following would be "nice to have's":

Put the ID on the <td> rather than the input
This would give us better control of the entire line.

Choose a better ID
ID's of the form type_# or probably too generic and will clash with some module eventually.  So if you use something like eue_type_# you aren't likely to bang into someone else's stuff.

Client Side Field Validation
certainly would be nice to have a hook to call javascripts for major events such as 'onchange' or 'onkeypress' on the input fields (of course then you would have to have an ID on the inputs!

Server Side Validation
Tougher to do - you would have to give us a field for php code to be eval'ed. or maybe a function call along with a php file (that would contain all validation functions) to be included.
